Abstract Hearing aids are widely used, and perhaps misused, daily by a large group of people with perceived hearing difficulty. The main reason for the use of these hearing aids is exposure to loud noise. In some cases, the exposure was caused by the nature of employment, for example gunfire on the part of servicemen.

Hearing aids are dispensed (sold) by groups offering audiology tests to determine the degree of impairment. Because income is based on the sale of hearing aids, the outcome of testing and the ensuing recommendations are skewed toward the purchase of high ticket hearing aids. The most glaring deficiency in this process is the lack of testing to determine speech recognition.

Abstract : Water quality and human health are inextricable linked. . The present study assessed the water quality index (WQI) based on physicochemical analyzed of six subsurface water sampling stations of different wells in east Wollega zone. This has been determined based on the physico-chemical water sample analyzed during past time when the well-developed measured at the water works design and supervision Enterprise laboratory of Addis Ababa. click here
For calculating the WQI, the following 12 parameters have been considered: pH, total hardness, calcium, magnesium, sodium, chloride, nitrate, sulphate, total dissolved solids, iron, manganese and fluorides. The WQI for these wells ranges from 8.484 to 372.2.  The high value of WQI has been found to be mainly from the higher values of iron, nitrate, total dissolved solids, hardness, fluorides and manganese in the subsurface water. The results of analyses have been used to suggest models for predicting water quality. The analysis reveals that the some subsurface water of the area needs some degree of treatment before consumption, and it also needs to be protected from the perils of contamination. The computed WQI for point W4’ w5 and windicates Excellent quality of water while the value of w2 (67.1) falls within (51-75) of the classification of water quality based on weighted arithmetic WQI method. It is observed that falls in poor quality, indicating subsurface water not fit for drinking purpose. So it follows that untreated water from the Gulfa is poor quality and must therefore be treated before use to avoid water related diseases. The WQI for the well point W1 and w3 ranges above 100 that indicate unsuitable for drinking. We already concluded that the WQI method is a very effective tool for determining the quality of water susceptible to leachate pollution and communicating it in an unambiguous manner to the stakeholders in the water industry. Keywords: Contamination, Parameters, Subsurface water, Water Quality Index, Weighted Arithmetic method.

Abstract : Objectives: Specialized imaging plays a major role in the evaluation of acute emergency conditions. The unnecessary radiologic studies pose a major challenge to the practicing emergency physician. The purpose of this study was to assess the pattern of ordering specialized imaging from the ED. click here
Methods: This prospective, quality improvement study was carried out at the Royal Hospital (adult ED) in Muscat, Oman, from August 2014 to March 2016. A random sample of emergency doctors and other specialty doctors on duty were requested to fill a data sheet and provide justifications of ordering imaging scans from the ED, at the same time all radiologist on duty filled another sheet for the same patients independently. Results: Of the 275-paired forms, 496 sheets were analyzed and 27 forms were excluded. Emergency physicians requested 80% of the studies, and two-third of the requested studies were CT scans. Around 89% of the forms showed that the radiology study helped the requesting doctor in managing the patient regardless of study result. Agreement between radiologist and ordering doctors from the ED were almost 98%. Conclusions:  Majority of the requested imaging studies helped in the evaluation of acute emergency conditions. However, Improving the system will be achieved by wise clinical decisions minimizing unnecessary radiological exposure. Keywords: Workflow, ordering practice, quality improvement, unnecessary imaging, patient safety, overuse.

AbstractIntroduction: Electrocution caused by an electric blanket is a rare and challenging clinical condition. Limited numbers of Cases were reported; unfortunately, most of them were post-mortem and as a result of overheating. Our patient is an example of rare survival cases presented with electrocution and was due to forgetting the electric blanket turn on. Our case is unique as severe multi-organ damage was caused due to both excessive heating of the body and exposure to an electric shock. The damage was involved the heart, lungs, kidneys, bone marrow, musculoskeletal system and skin. click here
Case presentation: Our patient is the female of 74 years old she ives alone at home with limited mobility because of severe osteoarthritis in the back and knees as well as history of old fracture in the right shaft of the femur. She is long standing diabetes with neuropathic complications. She is Post Trans-catheter Aortic valve implantation [TAVI] for severe aortic stenosis. She was presented to the hospital due to generally unwell status following the events of electrocution due to electric blanket. She was very unwell, lethargic and hemodynamically unstable. The diagnosis was confused initially as pulmonary embolism and/or acute ischemic heart events due to her acute presentation with hypoxia, elevated d dimer and troponin in addition to the acute ECG changes. Further evaluation revealed the presence of multiples burn marks with an entrance wound in the right back and exit wound in the left heal. The diagnosis of multi-organ damage due electric blanket was established. The damage was extend to involve the bone marrow causing transient pause resulted in significant symptomatic anemia required blood transfusion. The patient treated conservatively and finally discharged home in good condition. Conclusion: verheating by Electric blanket can be highly risky and it can result in multi-organ failure extend to the bone marrow and result in significant anaemia.Diabetic patients are effected more due to neuropathic complications. Taking a proper thorough history and performing an early comprehensive systematic examination on the patients is essential for diagnosis. Keywords: Electric blanket; overheating; multi-organ damage; prosthetic valve; electrocution; bone marrow; blood transfusion.
